At the time of admission to hospital she also reported using crack cocaine 10–12 times per month. As the patient’s dysphagia progressed, she began to crush, heat, and inject her prescribed opioids. She was prescribed slow-release oral morphine (SROM) (60 mg three times a day) and immediate release oral morphine (10 mg four times a day) by her community addiction specialist as opioid agonist therapy (OAT). She had been using injection drugs for several decades, including heroin, crack cocaine, and fentanyl. Her esophageal stricture resulted in progressive dysphagia, and she was eventually unable to swallow any tablets including her anti-retroviral therapy. Her past medical history was complex in addition to HIV infection, she was known to have HIV-induced immune thrombocytopenic purpura, hepatitis C infection, chronic obstructive pulmonary disease, depression, recurrent facial cellulitis, and polysubstance use including opioid use disorder. She remained free of cancer recurrence but developed esophageal stricture as a late complication of radiation therapy. The patient was diagnosed with a squamous cell carcinoma of the oropharynx 2 years prior and received treatment with an extended course of radiation therapy. Case presentationĪ 58-year-old woman with a 30-year history of infection with human immunodeficiency virus (HIV) was admitted to hospital for management of dysphagia secondary to esophageal stricture. Utilizing a CADD pump for the purposes of reducing harm by providing a safer opioid supply was uncharted territory for our team. CADD pumps are typically reserved for the management of acute pain in the post-operative period, or for management of chronic malignant pain or dyspnea in patients with life-limiting diagnoses. The pump can also deliver patient-initiated bolus doses at pre-programmed time intervals, with a lockout period to prevent inadvertent toxicity. We report a case of a female patient in acute care with a longstanding opioid use disorder who received injectable opioid agonist therapy using a continuous ambulatory drug delivery (CADD) pump in order to facilitate completion of her hospital admission.Ī CADD pump contains a reservoir of medication, typically a parenteral opioid such as morphine, hydromorphone or fentanyl, and can be programmed to administer an automated hourly infusion of medication through an intravenous or subcutaneous line. Similarly, prescription opioids (oral and injectable) have been proposed as a treatment option in the acute care environment for patients with severe opioid use disorder. There is evidence that offering prescription injectable hydromorphone or diacetylmorphine (pharmaceutical-grade heroin) in the community setting for people with severe, refractory opioid use disorder improves health and social outcomes. The implementation of harm reduction strategies in the acute care setting may mitigate negative health outcomes. In Canada, harm reduction services such as distribution of sterile equipment, safe disposal of used equipment, supervised consumption services and distribution of overdose prevention kits are readily utilized in the community setting and not widely implemented in inpatient settings. Stigma from healthcare staff, hospital policies enforcing abstinence, suboptimal pain management, and withdrawal symptoms can often result in involuntary or patient-directed discharge, thereby driving poor health outcomes. Despite the availability of opioid agonist therapy (OAT) such as buprenorphine/naloxone (suboxone), methadone and naltrexone, a proportion of hospitalized patients may not respond, have access to or be accepting of these interventions for substance use. Harm reduction does not require abstinence from substance use rather it is meant to reduce the potential health and social harms associated with drug use. Harm reduction is a patient centered, evidence-based approach compromising one aspect of care for people with substance use disorder (SUD). ![]() ![]() If you lose the return envelope, mail the payment to Tri-County Electric Co-op, P.O. I left a lot of them in the article because I think getting a whiff of the show’s writing is one of the best ways to remember it, short of watching all these episodes. I took down every quote myself – the ones that made me laugh or struck me as clever. Over the course of a weekend I re-watched every episode here along with a few others that didn’t make the cut. However, I do think it serves as a solid cliff-notes version of the series, especially if you watch these episodes in chronological order. This article is for me, to get it out of my system and move on. Help me heal your heart hole! Ah, who I am kidding. If you’re just getting into Community – though this is apparently unlikely since critics have constantly complained that the show is alienating to new viewers – or if you’re a fan wanting to look back on some of the best moments, this list is for you. I had planned to do this article eventually, and now seems like the perfect time. What I can say for sure is that fans are left with three amazing seasons of television and at least one very good one to treasure as our own. ![]() Maybe with another season Harmon’s punches could have gained the power of kicks. Actually that sounds like exactly the kind of thing Community would do. It would have had to graft new parts into its battered frame like some kind of cyborg… huh. To capture a sixth season it would have had to reinvent itself in a much more dramatic way than it had done previously. Second, without Chevy Chase and Donald Glover and the characters they played, the show had lost a sizable chunk of the dynamic that made it what it was. Community is a super senior now, and eventually you have to put college behind you and move on (no one pitch Community: Grad School, please). First, this being a show about a community college (a mantra sometimes repeated in jest as the show grew further from those beginnings) it would make sense for them to have four years and then go their separate ways. The reasons I’m not sad, though, are two. It’s just hard to see it come this far and fall short of its arbitrary, self-established finish line. The show probably got at least one or two more chances than it could have expected. But it didn’t – it pressed on through the valley of season 4 and creator Dan Harmon’s absence, and then miraculously into a fifth season. Had it ended after the season 3 finale, it would be held in Firefly-like esteem. Unfortunately, Community really did neither. Sometimes it’s better to burn out than fade away. To understand that, you must only understand that my favorite television show of all time is The X-Files, and we all know what happened when that show lost important cast members and trudged on for two additional seasons. I’m upset that the show is cancelled – both because that means that the prophecy of Six Seasons and a Movie will likely never come to pass, and also because it means that yet another great television show was comprehensively under-seen by the public – but I’m not so much sad. In truth, though, perhaps tales of my grief have been exaggerated. I hope that if I should die suddenly this week my friends will stage it to look like a suicide brought on by the sudden unjust cancellation of Community. The Abed character was something special that I think really spoke to people. It made it okay to use movies and TV as ways of expressing yourself and connecting with the people around you. It was a colorful, creative show that pushed and sometimes destroyed the boundaries of what could be done in a half hour network slot. In some ways it’s like the show was made for me. Over the past year or two as I went through the older seasons on DVD and watched the new ones on TV or on Hulu, the show has come to eclipse even 30 Rock and Arrested Development to take its place as my favorite TV comedy. NBC decided not to renew that weird sitcom it’s been airing before Parks and Rec, the one with the rabid cult fan base and low ratings (though, it should be noted, not much lower than Parks). We got the news on Friday that Community was done. ![]() |
